The PACE Social Worker provides direct case management services to participants. Responsibilities include participant screening and assessments, social service support, counseling, and referrals.

Other Information:

• Master's degree required
• LMSW or LCSW required
• Two years experience in healthcare preferred
• Experience working with the frail and elderly population preferred
• Excellent customer service skills

This job will be authorized 80.00 hours bi-weekly.
