Role Description

This is a full-time hybrid role for a Case Manager, based in Jakarta, Indonesia, with the flexibility to work from home part of the time. The Case Manager will be responsible for coordinating and managing patient care plans, assessing patient needs, and facilitating communication between patients, families, and healthcare providers. The role also involves monitoring patient progress, ensuring the quality and continuity of care, and advocating for patient well-being. A focus on compliance with healthcare regulations and hospital policies will be integral to this role.

Qualifications

  • Strong interpersonal and communication skills, with experience in patient advocacy and counseling
  • Proficiency in healthcare management and care coordination
  • Knowledge of healthcare regulations, compliance policies, and resource utilization
  • Ability to work effectively in hybrid work environments and collaborate with multidisciplinary teams
  • Proficiency in using digital healthcare tools, scheduling systems, and health information management platforms
  • Previous experience in case management, social work, or healthcare-related fields is advantageous
  • Bachelor's degree in Nursing, Healthcare Management, Social Work, or a related field
  • Fluency in English; proficiency in Bahasa Indonesia is preferred
